Life Transformation Counseling partners with Therapy Appointment Network to provide clients online appointment scheduling services.

All CURRENT clients may use the Therapy Appointment tab below to check availability and book an appointment with your counselor.  Click the “Therapy Appointment” button below to sign into your Patient Portal account for scheduling. If you have forgotten your username or password please contact the office.

All NEW clients, may call us at (813) 575-8199, if we are unavailable to take your call, you may leave a confidential message. Life Transformation Counseling will return your call within one business day. Please be advised that our identifying information may appear on your Caller ID.

Prior to your first appointment, you must complete all of the intake forms. LTC will send you the documentation needed via THERAPY APPOINTMENT. Should you not be able to complete the forms in advance, please arrive 30-minutes early to your initial appointment to complete these forms at the office. Please remember to sign where indicated.

NOTE: If client is a minor, both parents must sign the consent form for treatment. However, if you have legal court documentation that states otherwise, please bring it in with you during your first appointment.
