Payment Information
Rates
The price of therapy varies per counselor. Appointment fees ranges between $50 - $160 per 50-min session.

Insurance
Life Transformation Counseling counselors are considered an “out-of-network provider”. Essentially, this means clients pay Life Transformation Counseling directly for services provided and then seek reimbursement from their insurance company. Services may be covered in full or in part by your health insurance or employee benefit plan. If your plan does provide reimbursement, we will be happy to provide you with the necessary documentation to submit to your insurance company. Please check your coverage carefully by asking the following questions:
•Does my insurance plan provide coverage for mental health services?
•What is my deductible and has it been met?
•How many sessions per year does my health insurance cover?
•What is the coverage amount per therapy session?
•How much does my plan reimburse for out-of-network providers?
•Is approval required from my primary care physician?
Life Transformation Counseling cannot guarantee coverage or reimbursement under your insurance plan.

Reduced Fee
Reduced fee services are available on a limited basis. You may address this during your first session with the therapist. After your initial session, you will need to provide the appropriate paperwork in order to receive approval. Reduced rate services may be approved depending on financial need and reduced rate availability. * Reduced rates are limited and not guaranteed!
Cancellation Policy
Your appointment time was reserved just for you. Kindly notify us via e-mail or by phone 24-hours in advance if you need to reschedule your appointment. Cancellations that are not done 24 hours prior to the date and time of the scheduled session will result in a full service charge/fee. The credit card that is kept on record will be charged for the missed appointment.
No Show Policy
If you do not attend your scheduled appointment, it is considered a “no show.” No Shows will result in a full fee service charge. The credit card that is kept on file will be charged for the “no show.”

Good Faith Estimate For Mental Health Care, Consulting, and Service

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. .
