Payment Information

Appointments are billed at $120 per 50-minute session for Individual, Couple, and Family Therapy.  Payment is due at time of the appointment.

Life Transformation Counseling counselors are considered an “out-of-network provider”. Essentially, this means clients pay Life Transformation Counseling directly for services provided and then seek reimbursement from their insurance company. Services may be covered in full or in part by your health insurance or employee benefit plan. If your plan does provide reimbursement, we will be happy to provide you with the necessary documentation to submit to your insurance company. Please check your coverage carefully by asking the following questions:

•Does my insurance plan provide coverage for mental health services?
•What is my deductible and has it been met?
•How many sessions per year does my health insurance cover?
•What is the coverage amount per therapy session?
•How much does my plan reimburse for out-of-network providers?
•Is approval required from my primary care physician?

Life Transformation Counseling cannot guarantee coverage or reimbursement under your insurance plan.

Natalie Southward is a provider for the Children’s Board Administrative Services Organization (ASO).

Reduced Fee

Reduced fee services are available on a limited basis. You may address this during your first session with the therapist. After your initial session, you will need to provide the appropriate paperwork in order to receive approval. Reduced rate services may be approved depending on financial need and reduced rate availability.

Cancellation Policy

Your appointment time was reserved just for you. Kindly notify us via e-mail or by phone 24-hours in advance if you need to reschedule your appointment. Cancellations that are not done 24 hours prior to the scheduled session will result in a full service charge fee. The credit card that is kept on record will be charged for the missed appointment.

No Show Policy

If you do not attend your scheduled appointment, it is considered a “no show.” No Shows will result in a full fee service charge. The credit card that is kept on file will be charged for the “no show.”
